Referrals
People are able to self-refer to us directly by contacting us, but if you are a GP, health professional or other social care professional, or employer wishing to make a referral on behalf of someone with their consent, please download and complete the SSP Referral Form then email it to us at sleepprojectmanchester@gmail.com
We aim to contact the person within 5 working days of receiving the completed referral form.

What information can be shared with the referrer?
Due to confidentiality, we are unable to share any details of the support offered to the client, or any information about what is being addressed throughout counselling, or other information regarding the counselling process.
Why do I need signed consent?
Due to GDPR and confidentiality, we need signed consent to process any person’s information and to contact them to proceed with the counselling referral process.
Who can make referrals on behalf of someone seeking support?
Referrals can only be accepted from GPs or other medical/health professionals or social care professionals, and employers.
We are unable to accept referrals for someone from their family members, friends or partners.
